Covid-19 Screening Protocol and Precautions for Patients and Staff
As of 11/2/20, we are only allowing the patient into the office.
When you arrive for your appointment, please remain in your vehicle and call the office (219-736-8118) to let the staff know you are here. When the staff is ready for you, we will come out to escort you into the office. At this time, we will go through the COVID-19 Screening Protocol, check your temperature and evaluate you for the following symptoms:
Cough, Sore Throat, Congestion, Body Aches, Fever, Shortness of Breath, etc.
If you have a temperature or any symptoms listed above, you will be asked to return home for self-care and we will call you to reschedule your appointment.
If you are fever free and symptom free, you will be escorted into the building and asked to wash your hands before signing paperwork. Masks are required for the entirety of your visit.
If you have been tested for COVID-19 recently, please inform us of the date of the test as well as your results.
If you have been driven to your appointment by someone else, we ask that they remain in the car for the duration of your visit. Due to strict COVID-19 precautions, there can be no one in our waiting areas at this time.
